For Clients: Please be sure you have downloaded and completed the Client Health History forms as well as the Client Bill of Rights for the specific practitioner you will be seeing. Please bring all forms to your first appointment.

For Class Participants: Be sure you have reviewed the items you need to bring to class and the specific location of your class (which may be different than our office).

Cancellation Policy

In consideration for all of you, our clients, who are waiting to get an appointment: if you cannot make an appointment, you will need to notify us a minimum of 48-business hours in advance (2 full business days ahead of your scheduled appointment) to avoid being charged for the entire appointment. We thank you for your help and cooperation with this delicate issue.
